Harrison Abstract The potential consequences of crash involvement in the Psychological domain are discussed, based on the small corpus of relevant literature and additional material concerning psychological disorders thought to occur as a result of exposure to traumatic events. The results of an analysis of Victorian crash data suggest that the lifetime risk of developing psychological disorders resulting from injury in a crash is quite high. For post traumatic stress disorder, for example, this risk is estimated at 13.1%. It is concluded that there is a need to conduct research in this area to investigate the psychological consequences of crash involvement in the Australian context and to develop and evaluate therapeutic interventions which might reduce the impact of crash involvement. This report is the result of concerns about the lack of interest in the likelihood that the psychological consequences of crash involvement may have a significant long-term impact on crash victims. 'Re possibility that there may be substantial social and medical/paramedical costs associated with psychological trauma has not been investigated in the Australian context, and there is relatively little research in this area in the international road-safety literature. This report presents a general overview of the involvement of psychological processes in the crash event and recovery, and then focuses on the likely development of psychological disorders as a result of crash involvement. Four specific disorders are discussed, and using information drawn from the literature, crash data from Victoria, and the 1996 population census, estimates of the lifetime risk of developing each disorder as a result of injury in a crash were calculated. Extractions: Editorial TOWARDS A CAMPAIGN TO COMBAT PSYCHOLOGICAL DISORDERS IN THE COMMUNITY Eiad Al-Faris, MBBS, MSc, MRCGP Studies have shown a high prevalence of mental disorders (MD) among patients in primary health care (PHC) centers worldwide, and these disorders are of public health importance because of the associated increased disability. In Saudi Arabia, the prevalence of MD among consecutive patients attending their PHC physician was estimated to be 30%-46%. A study in Saudi primary health care centers that involved 2637 patients has shown that more than 90% of probable cases identified by a screening instrument, "The General Health Questionnaire," were missed. None of the 28 doctors who participated in the study had received structured training in Family Medicine or Psychiatry. The majority of medical practitioners in the Arab world have been biomedically trained to think of the human being as an assemblage of parts and particles and in their diagnostic hypotheses are usually confined to physical-organic causes only; the possibility of mental disorders is either not considered, or made later after excluding the probable organic causes (diagnosis by exclusion). Patients seen by PHC physicians commonly have neurotic disorders (mainly anxiety and depression), insomnia, marital disharmony and grief. In contrast to the psychotic patient who is taken to the hospital by his relatives, neurotic patients seen in PHC continue to maintain their social roles.

Mood Disorders include Depression, Mania, and Manic-Depressive (Bipolar) Disorder. A Mood is difficult to define, but moods are a common aspect of human experience. Depression is not about what kind of mood one is in. Depression is about the absence of an emotional response capacity. In the case of Manic-depressive, or Bipolar disorder, the question or issue is more one of lack of control of ones moods and the incapacity of ones psychological system to link moods (abiding emotional states of being) appropriately with thoughts. I hesitate to say that moods are a normal part of human experience. It is difficult to determine whether ones mood is normal are not. Such is particularly the situation with the person who experiences early onset of depression. First of all, one does not consciously know that he or she has a mood disorder. The situation is similar to needing eyeglasses and not knowing it; or having diabetes or hypertension and not having any manifest signs of the disorder. In the case of depression, one can grow up thinking that the absence of emotional responsiveness, or a certain listlessness or hopelessness is normal.

Methods for answering the question: To answer this question, survey researchers are trained to administer short diagnostic tools which suggest whether or not an individual is psychologically impaired. However, these survey research tools only "estimate", but do not actually diagnose, one's psychological state of being. Next, a stratified random sample of the population is identified. Large research studies (e.g. those conducted by the National Institute of Mental Health, NIMH, or the Centers for Disease Control, CDC) divide the U.S. into sections or geographic regions. Within each region subsections are identified to represent rural, suburban, and urban zones. Finally, individuals will be identified within each zone who represent different gender, race, age, religion, ethnicity, etc., groups. Researchers then interview the identified individuals with the hope that the sample is a good representation of the entire population. Typically, only federal agencies and large universities with federal grants have the resources necessary to collect this kind of data. A smaller study gives a less complete answer, so it is important to look at whether results of several small studies
