Extractions: Conversion Disorder Project (restricted access) Conversion Disorder is a common problem in clinical neurology. In Motor Conversion Disorder there is partial or total paralysis of one or several body parts; the clinician judges that the symptom is involuntary and there is no evidence of conventional neurological pathology on examination or investigation. Although the diagnosis of Conversion Disorder should be made with care, recent studies indicate that the rate of misdiagnosis is low in contemporary practice. There must be some neurobiological explanation for paralysis seen in Conversion Disorder. At the very least there is a failure to activate cortical motor regions which directly drive movement. But whether this results from impaired activity in premotor centres, or via suppression of motor processing, is unclear. This question should be susceptible to study with functional imaging. Extractions: Important Terms The Somatoform Disorders take on the form of the soma, or body. All are characterized by somatic complaints without physical basis. In contrast to malingering and other forms of faking, for these persons the illnesses, pains, and physical complaints are believed rather than feigned. Somatization Disorder (Briquet's) was originally described by Briquet. Person with this disorder present with multiple complaints of physical ailments over a long period. These begin before age 30, are inadequately explained by independent physical findings and that lead to medical treatment or significant life impairment. The complaints are are typically dramatic and significant; hence, they often lead to major medical intervention, such as surgeries. When performed these interventions tend to lead to a conclusion that no physical basis can be found. When doctors refuse treatment these person tend to "doctor shop" until they find someone new who will listen to their complaints. Diagnosis requires reporting of symptoms in four categories: Pain Gastrointestinal Sexual Pseudoneurological Caution should be used in assuming that no physical basis underlies complaints for these persons. Follow-up studies have shown that for a significant number of Briquet's patients some basis for a portion of their complaints may later be found.

I know of no assessment scale for this and doubt whether there has been much work conducted in this area. Research has shown some evidence for genetic as well as environmental factors may play a role. Symptomatology: Includes a history of physical complaints prior to age 30 which occur over a period of several years. There must be a significant impairment in functioning or a history of resulting medical treatment. After appropriate assessment by a physician, there is a lack of explanation for the reported symptoms or for at least the severity of the complaints. Treatment: Treatment typically includes long term therapy. The involvement of a single physician is important as a history of seeking medical attention and doctor shopping is common. Prognosis: Is typically a chronic condition with a variable course. Individuals with this disorder do not experience any significant difference in mortality rate or significant illness. Conversion Disorder Etiology: Two schools of though prevail in the etiology of this disorder. One is the psychoanalytic theory which posits that unconscious conflicts cause the conversion of this conflict into physical symptoms. The other suggests that a disturbance in the central nervous system results in this disorder, but neither theory has shown strong statistical evidence.
