This break in the anal lining often reopens during bowel movements. The result is bright red blood and intense pain. What is going on in the body? An anal fissure is a tear in the bowel lining. It causes painful, bloody bowel movements. It is often confused with hemorrhoids, which are painful swellings at the anus caused by enlarged veins. A fissure can be acute or chronic. The chronic condition is often associated with a buildup of tissue at the external end of the tear. This is called a sentinel pile and may be noticed by an affected person. The internal end may also have a buildup of tissue that an affected person can rarely see. When a fissure is present, the mucosa of the anus opens each time it is stretched to allow for a bowel movement. This continual opening prevents healing. It can also leave scar tissue. Most chronic fissures are in the center of the back of the anus. The anal sphincter muscles need to relax for a person to have a bowel movement. CAUSE Anal fissure can result from passage of large, hard stools or may be a secondary result of anal surgery, proctitis, or other diseases. Since the surface of the skin at the anus has been worn away, defecation irritates the fissure and may cause the sphincter muscles to spasm, resulting in intense pain. Fissures are often associated with constipation or inadequately emptied stools, and may also be linked to stress or hemorrhoids. DIAGNOSIS During a physical examination to locate the source of pain, the fissure can usually be seen, although anoscopy, in which a small plastic or metal instrument called an anoscope is inserted into the lower few inches of the rectum, may be used to confirm the findings. Flexible endoscopy is also useful in establishing a diagnosis. TREATMENT Anal fissures are treated with warm baths and topical creams; stool softeners (brans such as psyllium) are added to the diet. Surgery is generally unnecessary but may be recommended in severe cases. After the fissure healswhich may take 2 to 3 weeksa sigmoidoscopy may be performed to check for other rectal problems.
