Open Lipped Schizencephaly Findings:
A large CSF intensity cleft extends from the left lateral ventricle to the frontal cortex, lined by gray matter. Sulcation is abnormal on the right, with thickened cortex and pachygyria. Differential Diagnosis:
schizencephaly versus porencephaly. Lining by gray matter distinguishes between the two. Discussion:
-schizencephaly is the most common migrational anomaly occurring at approximately 6 weeks gestation.
-other migrational anomalies: lissencephaly, polymicrogyria, pachygyria
-infarct vs. developmental, TORCH

Schizencephaly What is Schizencephaly? Schizencephaly is an extremely rare developmental disorder characterized by abnormal slits, or clefts, in the brain's cerebral hemispheres. Schizencephaly is a form of porencephaly in which there is a cyst or cavity in the cerebral hemispheres. Individuals with clefts in both hemispheres (bilateral clefts) are commonly developmentally delayed and have delayed speech and language skills and corticospinal dysfunction. Individuals with smaller, unilateral clefts (clefts in only one hemisphere) are often paralyzed on one side of the body and may have normal intelligence. Patients with schizencephaly may also have varying degrees of microcephaly (abnormally small head), mental retardation, hemiparesis or quadriparesis (partial or complete paralysis), and reduced muscle tone (hypotonicity). Most patients have seizures. Some may have hydrocephalus. Is there any treatment?

When my daughter was a 8 months old we noticed that she was having some delays in her motor skills. We took her to the neurologist and her diagnosis was 1. cortical dysplasia involving the right per-sylvian region with broad gyri which are diminished in number associated with thickened cortex. There is deep enfolding of the cortex with a deep sulcus which nearly reaches the lateral ventricle. While this is reminiscent of closed lip schizencephaly, a thin band of white matter separates the enfolded gray matter from the ventricular body. 2. Absence of the septum pellucidum. 3. The optic chiasm and nerves appear to be within normal limits for size.
She is now 5 years old and only has a lack of motor skills involving her left arm and leg. She has not shown any other problems but with this diagnosis should I be concerned about any other problems that may come up in the future. And is this hereditary.

*For Medical Professionals only, registration required Schizencephaly, a grey-matter-lined cleft that extends through the entire cerebral hemisphere from the ependymal surface of the lateral ventricle to the cerebral cortex. Two types of schizencephaly are recognized: a "closed-lip" schizencephaly in which the cleft walls are contiguous and an"open-lip" schizencephaly in which the walls are separated by wide subarachnoid spaces. The grey matter lining the cleft is usually polymicrogyric (see polymicrogyria ). The clefts can be unilateral or bilateral, symmetrical or asymmetrical and can appear anywhere in the brain. These lesions are believed to be due to in utero vascular injuries and their preferential location in the perisylvian areas follows the frontoparietal distribution of the middle cerebral artery.

*For Medical Professionals only, registration required Schizencephaly, neuronal migration abnormality which occurs early in gestational life, possibly around the 7th to 8th week. It may be related to a vascular event. The resulting abnormality is a seam of grey matter extending from the pial surface of the brain down towards the ependyma of the lateral ventricle. This results in a grey matter lined cleft which can be described as open lipped, where there is a cerebrospinal fluid (CSF) track between the two sides, or closed lipped, where no CSF is identified. There is usually an associated abnormal contour of the lateral ventricle where the cleft meets the ventricular surface. In a large proportion of patients (70-80%) there will also be an absence of the septum pellucidum ( Fig.1

Summary : Schizencephaly is a rare congenital anomaly of the brain, characterized by formation of abnormal unilateral or bilateral clefts in the cerebral hemispheres. It often manifests with partial seizures, mental retardation and hemiparesis. Only two cases of schizencephaly associated with psychosis have been reported in the literature. A patient of schizencephaly, who had bipolar affective disorder is described. It has been compared with the earlier two reported cases of schizencephaly associated with pyschosis. Patients with schizencephaly may also have varying degrees of microcephaly (abnormally small head), mental retardation, hemiparesis or quadriparesis (partial or complete paralysis), and reduced muscle tone (hypotonicity). Most patients have seizures. Some may have hydrocephalus. Treatment for individuals with schizencephaly generally consists of physical therapy, treatment for seizures, and, in cases that are complicated by hydrocephalus, a shunt (a surgically implanted tube that diverts fluid from one pathway to another).

Patients may present with seizure disorders, mild to moderate developmental delay and mental retardation. Etiology/Pathophysiology
Failure of the normal migration of neurons from the germinal matrix zone at 1-5 months of gestation. A gray matter lined parenchymal cleft develops which extends from the subarachnoid space to the subependyma of the lateral ventricles. Pathology:
The cleft may be narrow or closed (type 1) or wide and open (type 2). It is frequently associated with complete or partial absence of the cavum septum pellucidum in approximately 80-90% of cases. the cleft is usually located near the pre- or postcentral gyri. The clefts can be unilateral or bilateral. Polymicrogyria, microcephaly, and gray matter heterotopia have also been noted. Optic nerve hypoplasia has been noted in 33% of cases, with possibility of blindness. Prognosis is poor, with severe intellectual impairment, spastic tetraplegia and blindness. Miscellaneous
Most brain malformation can be produced by a variety of injuries during gestation. This includes infections, X-radiation, certain drugs, metabolic and genetic abnormalities.

I am looking for information on "Schizencephaly". It is a very rare disorder. We have been given very limited answers to our questions, because they are not sure of long term outcomes. My granddaugher is now 2 years old, she is having speed, physical and occupational therapy once a week. She is coming along very well she is about 3 months behind in development so far. Her speech is not very good as yet, hopefully that will get better with therapy. Any information would be very helpful to us. Thanking you in advance. Next Article

Diagnosis: Schizencephaly
Full history:
26 year old man with left frontoparietal open-lip schizencephaly with overlying polymicrogyria presents with refractory complex partial seizures. Clinical evaluation suggests seizures most likely arising from the left frontal region.
Radiopharmaceutical:
9.0 mCi F-18 FDG, i.v.
Findings:
A defect in the left frontoparietal cortex is noted corresponding to the open-lip schizencephaly on MRI. The left hemisphere is smaller than that of the right and there is enlargement of the left lateral ventricle as showed on MRI.
Discussion:
Schizencephaly, open- or close-lip, is associated with polymicrogyria/agyria in the adjacent cortex. Patients present with seizure, developmental delay, mental retardation and possible blindness if associated with optic nerve hypoplasia.
