Dear Mieke;
Still hard at work on your thesis I see. I hope you still ask only the easly questions because Nephrology is not my specialty. Lipoid nephrosis is the same as Minimal Change Disease according to our Nephrologist on call. This can also be referred to as "nil disease".
This information is provided for general medical education purposes only. Please consult your physician for diagnostic and treatment options pertaining to your specific medical condition. Sincerely;

NIL DISEASE - A disorder of the kidneys which largely affects the glomerulus , the blood filtering structure. This disorder is one common cause of nephrotic syndrome in children affecting 2 to 3 children per 100,000 population under age 16 in the US. Minimal change disease is also seen rarely in adults. The cause is unknown but may be related to an autoimmune illness. Risk factors include a history for a immune disorder, recent immunization, or a bee sting. Diagnosis is made by renal biopsy . Treatment include systemic corticosteroids which are usually quite effective in curing this disease. Other medications include chlorambucil and cyclophosphamide. In most cases, a moderate protein diet (1 gram protein per Kg body weight per day) will be recommended. Salt (sodium) restriction can be helpful to reduce swelling and vitamin D is usually supplemented.

    Minimal change disease
    Definition: A disorder of the kidneys that affects the structures (glomeruli) which include small capillaries surrounded by membranes through which the blood is filtered to form urine.
    Alternative Names: Minimal change nephrotic syndrome; Nil disease; Lipoid nephrosis; Idiopathic nephrotic syndrome of childhood
    Causes, incidence, and risk factors: Minimal change disease is one cause of nephrotic syndrome . It is named because under a light microscope the glomeruli appear totally normal. Under an electron microscope characteristic changes in the glomeruli can be seen, including the fusion of a portion of the epithelial layer.
    Minimal change disease is most common in children but occasionally occurs in adults. The cause is unknown, but the disease may be preceded by viral infection, allergic reactions or recent immunizations.
    Minimal change disease does not reduce the amount of urine produced. It rarely progresses to renal failure

    Blood tests and urinalysis reveal results typical of the nephrotic syndrome including high levels of urine protein, low blood albumin levels and high cholesterol. A renal biopsy and examination under electron microscope shows classic changes of minimal change disease. An immunofluorescence examination of renal biopsy tissue is negative.

    Background: Minimal-change disease (MCD), also known as lipoid nephrosis or nil disease, is the most common single form of nephrotic syndrome in children. It refers to a histopathologic lesion in the glomerulus that almost always is associated with nephrotic syndrome. It typically is a disease of childhood, but it also can occur in adults. Pathophysiology: It is postulated that MCD is a disorder of T cells, which release a cytokine that injures the glomerular epithelial foot processes. This, in turn, leads to a decreased synthesis of polyanions. The polyanions constitute the normal charge barrier to the filtration of macromolecules such as albumin. When the polyanions are damaged, leakage of albumin follows. The identity of this circulating permeability factor is uncertain, although it is postulated that it may be hemopexin. Some of the cytokines that have been studied in MCD are interleukin-12 (IL-12) and interleukin-4 (IL-4). IL-12 levels have been found to be elevated in peripheral blood monocytes during the active phase and normalized during remission. Interleukin-18 (IL-18) can synergize with IL-12 to selectively increase the production of vascular permeability factor from T cells. In addition, levels of IL-4 and CD23 (a receptor for immunoglobulin E) have been found to be elevated in peripheral blood lymphocytes.

  • Nephrotic Urine versus Nephritic Urine . The nephrotic urine contains a large amount of protein but does not contain elements indicating active inflammation such as WBCs and RBC casts. In contrast, the nephritic urine is suggestive of acute renal inflammation and will contain protein, RBC casts, blood by dipstick, and WBCs. The differential diagnosis can be narrowed based on whether a nephritic or nephrotic urine is present.
  • Nephrotic Syndrome.
  • General . Nephrotic syndrome is not a disease but rather the renal manifestation of multiple underlying causes. The primary disease may be renal in origin, such as minimal-change disease, or may be a systemic illness with renal manifestations, such as diabetes mellitus with nephropathy. Nephrotic syndrome may be related to glomerulonephritis and other causes of nephritis. Once active renal disease (nephritis) is no longer ongoing (that is, burned out), the patient may end up with nephrotic syndrome and a nephrotic urine. Definition Presentation

    Figure 1. Bacterial fruit blotch of watermelon In Queensland, the fruit blotch disease of watermelon has occurred sporadically over the last 25 years. We have regarded it as a minor disease but recently fruit blotch has shown that, given suitable conditions, it can cause severe damage. Since mid 1995, blotch has been noticed in the Ayr, Bowen, Chinchilla and Emerald districts. The severity of the disease is greatly modified by weather conditions and outbreaks are often associated with periods of wet, windy weather. Hail damage can greatly increase disease severity. In 1996, a second strain which also infects rockmelon and honey dew was identified and these crops must now be considered at risk from this disease.
