Narcissistic Personality Disorder: An Object-Relations Perspective

Freudian psychodynamic theory establishes the theoretical base out of which Object Relations arises to explain personality development and psychopathology in terms of the internalization of relationships formed with others. This paper will lightly review some of the main theorists of object relations, and examine some of the most notable differences among them as well as the unifying themes. Then an even lighter treatment of cognitive behavioral perspectives on personality and psychopathology, as we turn to a case study to examine the utility of each perspective to explain the Narcissistic Personality Disorder. Summers (1994) points out that when Freud (1914) recanted his theory of sexual trauma, to focus on libidinal drives as the etiology of psychopathology, the psychodynamic theoretical orientation shifted from external events to internal events (1915a). Repression of memories of infantile sexual trauma was no longer the primary factor in personality development, but rather, the reduction of biological tension, by the gratification of drives was seen as the prime motivaton in development. These biological drives are expressed psychologically as wishes. For Freud, it was the repression of these wishes that resulted in psychopathology (1915b). Freud's use of the word 'instinct' occurred relatively late in his writings (1915d). However the notion of what would come to be identified as the id-based instincts appeared in his earliest work as 'excitations', 'affective ideas', 'wishful impulses', and 'endogenous stimuli'. He acknowledged (1920) that he was retaining the term, 'sexual instinct', even though he had decided that it had less to do about sex, and more to a general life force, that he identified as Eros, "...which seeks to force together and hold together the portions of living substance...[and] which is directed towards objects"(1920, p.60).

Narcissus, as the Greek legend depicted, fell in love with his own reflection. Narcissistic Personality Disorder (or NPD) is a disabling disorder, which affects every part of the sufferer's life and the lives of those around them. It is classified as a Cluster B Personality Disorder in the Diagnostic and Statistical Manual of Mental Disorders Fourth Edition DSM-IV (American Psychiatric Association, 1994), and the syndrome is now believed to be much more than simply 'self-love'. We all possess specific personality traits, or characteristics, which often develop in adolescence and become 'set' by adulthood. However, when they become rigid or unchanging and begin to impact on everyday functioning they can lead to psychiatric symptoms. Individuals with personality disorders often blame other people for their problems, and this is especially true of people with NPD. bodyOffer(22058) According to the DSM-IV these individuals have an extreme sense of self-importance, so much so that they appear to have lost touch with reality. This has been referred to as a 'Delusion of Grandeur'. They are extremely demanding, expecting to be admired and praised for everything they do. There also appears to be a contradiction at work - they have an inflated sense of self-worth but constantly require others to reaffirm this - they believe that they are unique or 'special' and that everyone else must acknowledge this 'fact'. Other characteristics that are evident among people with NPD include: lack of empathy or feelings for other people, inability to love others (they can be great 'actors' though!), tendency to envy others for their possessions or abilities, tendency to feel bored and restless a great deal of the time, tendency to idolize people they feel they can get praise or justification for their behavior from yet on the other hand they can just as quickly 'drop' these people if they no longer serve their narcissistic purposes, and the fact that many NPD sufferers tend to be intelligent, manipulative, control-freaks who can be outwardly charming on the surface but cruel and cold should the situation or person not meet their very high expectations.

Narcissistic Personality Disorder (NPD) is a severely disabling psychiatric condition, characterized primarily by intense self-love, delusions of grandeur, an inability to feel empathy for another human being and chronic pathological lying. Sufferers have trouble maintaining relationships, are hyper-reactive to criticism, often feel mistreated and are very susceptible to developing addictive behaviors. The question that this article will address is: what causes this disorder? NPD was first recognized in 1980 by the Diagnostic and Statistical Manual of Mental Disorders (American Psychiatric Association), and over the ensuing twenty years has proven very difficult to study. There are several reasons for this, but the principal reason is that narcissists tend not to seek help for their problems. The other major reason is that even when they do seek help (e.g. in the form of therapy) health professionals find that they are a very difficult population to deal with. bodyOffer(22227) The bottom line is that many people with NPD do not believe that they could possibly HAVE a problem in the first place.
