Bandolier has been stimulated by a number of papers on different treatments for migraine to try and put together a league table of relative effectiveness. In an ideal world this would be created from all the available evidence from systematic reviews. The problem is that the systematic reviews either have not yet been done, or anyway Bandolier couldn't find any.
In these circumstances the choice is either to forget it, or to press on with what evidence is available. Bandolier , you will not be surprised to learn, chose the latter course. So this section comes with the health warning that it is not complete - it is a "Saturday night special" review. Perhaps migraine treatment, which is now a big industry, is one of those topics about which people should write to ask for a Health Technology Assessment.
Sumatriptan
A review of the controlled clinical trials of sumatriptan up to 1993 was published by Peer Tfelt-Hansen from Copenhagen [1]. It contained data from an impressive number of randomised studies of sumatriptan given by subcutaneous and oral routes, and with impressive numbers of patients. Data from these papers has been extracted to calculate NNTs.

Five studies of subcutaneous sumatriptan 6 mg against placebo were highlighted in the Tfelt-Hansen review, with over 2000 patients. The primary outcome measure was the effect on migraine headache. Patients rated their headache on a four-point scale (severe, moderate, mild or none) and the definition of a successful response was the decrease of a severe or moderate headache to mild or none.

Migraine: what is it and where are we in understanding it?

Migraine is a severe episodic headache that affects about 10% of the UK population. It is more common in women, with a female to male ratio of 3.5:1. It is estimated to affect some 16% of adult menstruating females and cost several tens of millions of dollars a year in its total economic impact. This should be contrasted against the fact that the disorder is at least 4,000 years old. In practice there are, broadly speaking, two types of migraine. The most common is called migraine without aura (Common Migraine) and this accounts for 85% of all sufferers. These are episodes of severe pain that may affect only one side of the head, although they may be bilateral, and which is usually described as throbbing although again this is not always the case. The signature of these headaches is that they are usually, but not always, associated with feeling sick in the stomach or being sensitive to light, sound or movement of the body. Typically the sufferer would wish to lie down in a a dark and quiet room and wait for the storm to pass. The second most common type is called migraine with aura (Classical Migraine). The

A developing migraine made the sunlight difficult to bear. So how did a person suffering from migraine, a condition that can cause debilitating pain, return after halftime to score the winning touchdown and earn Most Valuable Player status? The answer: He recognized the early warning signs and immediately took an effective drug to control his pain. Michael John Coleman, founder of Migraine Awareness Group: A National Understanding for Migraineurs, or MAGNUM , appreciates the challenges migraine sufferers face. Coleman has himself suffered with bouts of severe migraine headache pain since age 6. During an attack, he says, "I felt like I was being beaten up by a gang." Coleman's attacks used to last 72 hours or more; a couple lasted more than two weeks straight. "It was nightmarish, when I look back at it. I don't know if I could live through that again." Not Your Usual Headache More than 26 million Americans suffer from the neurologic disorder of migraine, according to the American Medical Association. No medical test exists for migraine, so the diagnosis is based on having some or all of the following symptoms:

Tension headaches are usually described as a continuous pressure pain or tightness of varying severity over the entire head, whereas migraine headaches are a severe, throbbing pain over one or both temples, or behind one eye or ear, and are often accompanied by nausea and vomiting. A migraine often starts on waking up in the morning, but can occur later in the day and can last hours to one or two days. In individuals with a form of migraine called classic migraine, visual symptoms described as bluriness, dazzling zigzag lines, blind spots or sensitivity to light occur just before and sometimes during the headache. While fatigue and stress can sometimes bring on both tension and migraine headaches, bright lights, noise and alcohol are specific factors that can trigger a migraine.

Migraine treatments have developed rapidly in recent years, thanks, in part, to a growing understanding of the genetic causes of headache pain. Medications have become more effective and faster acting. Indeed, some analgesics (pain relievers) can stop a migraine before the headache actually starts.
Certain medications like Zomig, Imitrex, and Amerge, should be used where a clear diagnosis has been made, however pain relievers are no longer the only option. Behavioral therapy teaches people how to control headache pain through relaxation techniques and lifestyle changes. Alternative migraine treatments include herbal remedies, chiropractic manipulations and even the use of dental appliances.
