Chronically painful feet are hard to ignore and sometimes even harder to return to a pain©free state. As a treating chiropractor, I have found that actively involving the patient in their care has made a big improvement in treatment success. If your practice is similar to mine, you have many patients asking questions about the cause of their chronically painful feet and how to care for them. Surprisingly, it is difficult to find a concise compendium of information on painful feet suitable for patient use. I decided to create a brochure for our office that has become quite a hit with our patients. Herewith are the important excerpts from our Patient's Guide to Plantar Fasciitis/Heel Spurs. The brochure is too long to print here, but use the following material as a template. Modify it as it suits your practice style and training. Here it is.

Ouch! Are aching arches, sore heels or swollen feet holding you back from your favourite sport? Finding comfortable athletic shoes is a challenge, especially if you're plagued by foot ailments such as Achilles tendonitis, painful arthritis, bunions or weak ankles. But you don't have to stay on the sidelines. Here's what the experts suggest: Achilles tendonitis Because it's caused by pronation (feet roll inward) or supination (weight falls on the outer edge of the foot), look for a shoe with a slight heel raise, a sturdy heel counter and stability to help control the back of the foot. Arch problems The shoe should fit the height of your arch comfortably. Pedorthist Jennifer Gould of London, Ont.., says the shoe should help stabilize since arch problems can result from overpronation (feet roll inward excessively). High-arched feet tend to be rigid and require extra cushioning because they don't absorb shock well.

Foot Ailment Description Treatment Achilles Tendinitis Irritation and inflammation of the tendon that attaches to the back of the heel bone.Achilles tendinitis can be caused by improper warm up or overtraining. Can be treated with ice, rest, aspirin or anti-inflammatory medications.Chronic pain or any swelling should be professionally evaluated. Bunions Misaligned big toe joints which can become swollen and tender, causing the first joint of the big toe to slant outward, and the second joint to angle toward the other toes.Bunions tend to be hereditary, but can be aggravated by shoes that are too narrow in the forefoot and toe. Surgery by a podiatric physician is frequently recommended to correct the problem. Hammertoe A condition, usually stemming from muscle imbalance, in which the toe is bent in a claw-like position.It occurs most frequently with the second toe, often when a bunion slants the big toe toward and under it, but any of the other three smaller toes can be affected. Selecting shoes and socks that do not cramp the toes will alleviate aggravation.

Heel Spurs can occur from a spur on the bottom or back of the heel. A "spur" is an enlargement or extra amount of bone that should not be present. Occasionally a connected tissue that runs from the heel to the ball of the foot (called the "plantar fascia") is involved with the spur at the bottom of the foot. "... able to walk without pain." A spur on the bottom of the foot is called a heel spur. A spur on the back of the heel is called a "pump bump." Some of these situations are inherited, and can also be a result of continued foot stress such as extra running and jumping. Flattening and lengthening of the plantar fascia may cause small tears. When it tears near the bone, a heel spur may develop.
Podiatric Treatment
(1) The use of insoles, physical therapy, foot taping, foot exercises and oral anti-inflammatory medication and injection. (2) In rare cases, the area is operated on. Occasionally, heel pain is also associated with other medical disorders such as arthritis (inflammation of the joint), bursitis (inflammation of the tissues around the joint), or neuroma (a growth resulting from a pinched nerve and other soft tissue growths). There are a variety of causes of heel pain. Heel pain can occur on the bottom of the heel or the back of the heel. The most common heel pain occurs in the bottom of the heel. Frequently this pain is worse first thing in the morning or after having been at rest. It seems to come out of nowhere or feel like a stone bruise after having been on your feet for long periods of time. Patients often describe the pain as being sharp like stepping on a "spur" or a thorn. The pain may radiate into the arch of the foot or up into the calf muscle. On occasion, leg or foot cramps accompany the condition. Often associated with a spur identified on x-rays, the condition may be referred to as "heel spurs." A common contributing factor is tightness of the calf muscles. The pain is caused by the pull of a very strong ligament that attaches into the bottom of the heel and fans out into the ball of the foot. This ligament, called the plantar fascia, acts as a bow string to help support the arch of the foot. It is like woven rope and does not stretch well. When something causes the arch to flatten, the ligament is stretched more than it can, and it pulls on the heel bone, causing pain. We call this plantar fasciitis. This pain often eases after a few steps as the ligament is forced to stretch. In severe cases the ligament can tear or rupture. Home remedies should include soaking your feet in Epsom salts and warm water, taking an over-the-counter anti-inflammatory, heel cushions or shoe inserts, and calf muscle stretching. Wearing a good supportive shoe is very helpful. Surprisingly, wearing a shoe with a 1" heel may ease the pain.

This is a side ( lateral ) view of the heel bone, and demonstrates a spur on the back ( posterior ) surface of the heel bone (calcaneus). This was asymptomatic (without pain or other symptoms). A tight Achilles tendon can cause this to form. It may be associated with pain at the point were the Achilles tendon inserts into the calcaneus.

was a popular early spur maker in the southwestern part of the United States. He was born near Magdalena, New Mexico on September 26, 1894. Blanchard was a true cowboy spur maker. He worked on various ranches in New Mexico and made bits and spurs as a side line. Many of his spurs made after the mid 1940's were marked with the town and state of manufacture. San Antonio, N. M. is one such mark. Datil, N.M. is another. The New Mexico spurs are very collectable. When work on one ranch ran out, he would move on, which explains why so many different New Mexico towns names are on his spurs. Blanchard moved to Arizona in the 1950's. He went to work for the Boquias Cattle Company as a ranch cook. During that time he made spurs with the Seligman, Arizona mark.
