A word about breast reduction in men
Gynecomastia is a medical term that comes from the Greek words for "women-like breasts." Though this oddly named condition is rarely talked about, it's actually quite common. Gynecomastia affects an estimated 40 to 60 percent of men. It may affect only one breast or both. Though certain drugs and medical problems have been linked with male breast overdevelopment, there is no known cause in the vast majority of cases.

What is gynecomastia?
Some men and boys have fat on their chests that makes it look like they have breasts. This condition is called pseudogynecomastia (false gynecomastia). It is not the same thing as gynecomastia.
What causes gynecomastia?
Gynecomastia is usually caused by changes in hormones at puberty or as part of aging. Hormones are chemicals produced by the glands of the body, such as the thyroid gland, the testes and the ovaries. Scientists believe that gynecomastia may be caused by changes in the balance of two hormones, estrogen and testosterone. In rare cases gynecomastia is caused by prescription drugs, over-the-counter medicines, illegal drugs, tumors or disease.

Male Gynecomastia Gynecomastia is a common disease of the male breast where there is a benign glandular enlargement. The disease, which can involve one breast or both, affects an estimated 40 to 60 percent of the male population. While some have a hormonal imbalance, there is no known cause in the majority of cases.
Obesity, and/or excessive uses of alcohol, marijuana and anabolic steroids may cause male breast enlargement. Surgical correction is very successful. Male breast reduction surgery involves a safe outpatient procedure that removes fat and/or glandular tissue. If excess glandular tissue is the primary cause of the enlargement, deposits are excised with a scalpel. The excision may be performed alone or in conjunction with liposuction. The goal of the gynecomastia procedure is to restore and enhance a flatter, firmer, more masculine chest contour. Surgery to correct male gynecomastia can be done alone or with other procedures such as eyelifts and tummy tucks. The procedure, when performed by a qualified plastic surgeon, takes a few hours, usually under general anesthesia, or in some cases, local anesthesia plus sedation. Complications are infrequent and unusually minor. Presurgery planning is extremely important for male gynecomastia. Patients will need to openly discuss their medical history. Some patients will have already seen their primary care dostor and may have had their hormonal system checked and an x-ray or mammogram taken. Once the surgeon knows how much fat and glandular tissue is contained within the breast, a surgical approached can be defined that best suits the needs of the patient.

Half a centimeter is a only a bit more than a 1/4 inch, an amount that's probably not noticeable to anyone but you. However, when you say nipple (rather than "nipples") and "hard," I start to feel that, for safety sake, your doctor should check this out. He/she should check if it's "normal" gynecomastia, which is usually described as "firm," "tender," "rubbery," or "fleshy" and is often on both sides (though it certainly can be only one side). By a vast majority, most cases of man boobs have no associated physical problem or cause. They tend to run in families. Certain medications and drugs can increase the chance of getting them, including anabolic steroids, insulin, certain non-Prozac type antidepressants (the tri-cyclics) and marijuana, so staying away from them or checking with your doctor about anything you are taking is a good idea. Then, too, there are some genetic syndromes associated with gynecomastia. How can you stop this? After your doctor says that what you have is simple gynecomastia, unless they're really bothering you or are noticeably large, ignoring them is the best path. They usually go away by themselves within two years or so.

It is relatively common in adolescence, and usually disappears in a few months. But in about 10% of young males it does not. The culprit in gynecomastia is the female hormone estrogen. Description of Gynecomastia Excision Surgery Using Tumescent Liposuction and Scissors Dissection Technique

Surgical Technique
After sedation, a generous amount (a liter or often, two) of tumescent anesthetic is placed into the pectoralis muscle, the fat surrounding the breast tissue, the axillae, and the breast tissue itself, with a blunt rainbird infiltrator and a pump.  Depending on what other surgeries the patient is having on that day (and the total lidocaine load), an increased concentration of lidocaine is usually employed, commonly 0.15 or 0.2% with 1:300,000 epinephrine instead of 1:1,000,000 epinephrine.  It is advisable to wait for 15 minutes before surgery. We then test the gynecomastia tissue by liposuction with a 3-4 mm “candy cane” cannual (KMI, Anaheim, CA).  If the tissue responds, sharp surgery may not be optimal.  If sharp surgery is elected, liposuction is performed in two distinct planes:  the first plane above the breast tissue and below the dermis, with the second plane above the pectoralis fascia and below the breast tissue.  If there is very little fat, this may be done without suction to establish the dissection planes.

Gynecomastia means enlargement of the male breast. The definition has nothing to do with what causes the enlargement or what tissue it is composed of. It is most common around puberty and can involve one or both sides. Most commonly the enlargement is centralized in the breast but occasionally it is not. It is present to some degree in about a third of male population. The majority of cases are physiological or drug induced. Drug induced gynecomastia just requires cessation of the causative drug. If the gynecomastia is resistant to treatment or is the adolescent type but persistent surgery is required.
The type of surgery depends on the type of tissue contributing to the
breast enlargement.

While the surgery may be performed on men of any age, it is discouraged for those who suffer from obesity, as well as for men who drink alcohol excessively and/or smoke marijuana. Gynecomastia extracts excess fat and tissue from the breast to produce a flatter, more masculine shaped chest. The Surgery A surgeon may perform the procedure using either a scalpel, liposuction, or a combination of the two. In the first technique, an incision is made, typically in the underarm. The surgeon then extracts surplus tissue and fat from the chest region and stitches the incision. If, however, the gynecomastia is to remove mostly fatty tissue, then the surgeon may opt for liposuction. This technique involves a slim hollow tube (a cannula) that sweeps through the layers of the chest, breaking up the fat and suctioning it away. Once the fat and tissue is removed, the surgeon stitches up the incision and trims away any excess skin. These procedures last one to two hours and are typically performed on an outpatient basis. After the Surgery Following surgery, the incisions are dressed and the chest is wrapped in an elastic bandage. A small tube is sometimes used to drain away excess fluids. The surgeon generally removes the stitches in seven to fourteen days, while the bandage stays on for up to a month. Patients can generally return to work in two weeks time.

The word gynecomastia literally means woman's breast and the enlargement can be in both projection and width. How often does gynecomastia occur? The medical literature reports a wide range of incidence rates for gynecomastia and whether it occurs in both breasts or not. Gynecomastia rates peak at three distinct times of life; 60-90% of newborns can manifest gynecomastia. During puberty gynecomastia can occur in up to about 65% of males and a third peak of incidence occurs in adulthood. My fifteen yr. old son was recently diagnosed with Gynecomastia. He has a tender, enlarged right nipple. I have read about the connection between gynecomastia and testical cancer and am concerned. The doctor did not perform an exam of the testies. I have instucted my son to perform a self exam regularly. My question is: What are the other signs/symptoms of testical cancer and should I have my son return to the doctor now for a testical exam?
