The onset is usually seen early during puberty and is caused by hormonal imbalance with high levels of female hormones as compared to male hormones. This in turn leads to a proliferation of the breast tissue on one or both sides.
The incidence of gynecomastia during puberty is reported to be as high 50 to 60 %. By the end of puberty, when the hormonal metabolism has reached a balance, most boys will have breasts of normal size. In some cases, though, the enlarged breast tissue may not involute and the breast tissue will stay enlarged.
Apart from adolescent gynecomastia, the disorder is also seen at later stages in life. In these cases a full medical check up has to be performed to exclude conditions such as liver disease, endocrine gland malfunction or breast and other cancers. Drug induced gynecomastia must also be considered (steroids) as must chronic marijuana and alcohol consumption.

Smokers should stop smoking for a minimum of two weeks before surgery and during recovery. Smoking decreases blood circulation and can decrease proper healing. Overweight patients should first be encouraged to correct the problem with exercise or weight loss.

Some types of liver disease and cancers of several types can produce gynecomastia. Some cases are due to congenital syndromes and others may be related to drug use such as steroids or marijuana. There is a second peak in incidence late in life and here the risk of ductal carcinoma of the breast must be a stronger consideration so that biopsy of the breast may be necessary. 4. Yes No When gynecomastia is severe, when it persists, and in those cases where there is supicion of malignancy surgical treatment may be indicated. In most cases the incisions for this operation can be placed just inside the areolar border where they are less likely to be conspicuous. The glandular tissue must be removed. This firm fibrous tissue extends from the nipple areolar complex down to the surface of the pectoral muscle beneath. In some cases it is helpful to use liposuction to remove some of the fatty tissue which always surrounds this ductal tissue. Without this liposuction some patients would have a "donut" shaped defect after removal of the ductal tissue. Do men get breast cancer?
Many people are unaware that men can develop breast cancer. Every year in this country approximately 1000 men will develop carcinoma of the breast ( ). These men frequently experience surprize, confusion, and a sense of isolation. While the average age at diagnosis is around 65 years, the problem can occur in younger (or older) men ( How common is it?
For every 100 women who develop breast cancer in the US, one male is diagnosed with the equivalent disease. The overall incidence is between 0.1 and 3.4 cases per 100,000 man-years. This makes male breast cancer one of the rarest malignancies, contributing to the generally low level of public awareness. What causes it?
Just as the causes of female breast cancer are still under study, the precise cause (or causes) of male breast cancer are still under study. Men have glandular breast tissue that is subject to hormonal influences. Excess estrogen, especially around the time of puberty, has been identified as a possible factor. Men with Klinefelter’s syndrome have an increased risk of developing breast cancer, as do men who take estrogens or estrogen-like compounds. Androgen (and possibly progesterone) exert a protective influence. Men who are deficient in androgen seem to also be an increased risk (for example, men who have testicular atrophy from mumps orchitis, injury, or undescended testes). Brain tumors and conditions associated with excess production of prolactin have also been implicated in some cases of male breast cancer (

Gynecomastia, or excess development of breast tissue, occurs in as many as 40 - 60% of men. The cause is not always known, but the incidence increases with certain drugs (digitalis, anabolic steroids, Tagamet, and spironolactone), use of marijuana or alcohol, and obesity. He went on to describe how he followed a cycle of anabolic steroids and then afterward noticed a swelling of his chest that he thought was the muscle growing. It can occur naturally or as the result of certain substances being in the system. Historical writings and artifacts show that Gynecomastia is an ancient problem, as evidenced by statues of Pharaoh Seti I that have enlarged breasts. Aristotle reported encountering the problem, and the earliest description of the surgical removal of Gynecomastia is found in the writings of Paulus of Aegineta, who lived from 635 to 690 A.D.

The condition may occur in one or both breasts and begins as a small lump beneath the nipple, which may be tender. The breasts often enlarge unevenly. Gynecomastia during puberty is not uncommon, is self-limiting, and usually goes away over a period of months.
In newborns, breast development may be associated with milk flow ( galactorrhea ). This condition usually lasts for a couple of weeks and in rare cases may persist until the child is two years old. It is caused by exposure to maternal hormones.
Other causes of gynecomastia include chronic liver disease , kidney failure, and exposure to estrogens, androgens (often taken secretly for body building).

Indications
This operation is to remove excess breast tissue in men. This develops in one of two patterns, either a firm button of tissue beneath the nipple or a diffuse fatty change with poorly defined edges. A surgery that removes fat and or glandular tissue from the breasts, and in extreme cases removes excess skin, resulting in a chest that is flatter, firmer and better contoured. What does it accomplish? The chest is flatter, firmer and better contoured. What happens during the first consultation? Dr. Adams will discuss your surgery and determine if you are a good candidate.
