Goiter: enlargement of the thyroid gland, causing a swelling in the front part of the neck. Hyperthyroidism: excessive thyroid gland activity, marked by increased metabolic rate, goiter, and disturbances in the autonomic nervous system and in creatine metabolism; sometimes used to refer to Graves' disease. This metabolic imbalance results from Excessive thyroid Hormone. The most common form of hyperthyroidism is Graves' disease, which increases T 4 production, enlarges the thyroid gland (goiter), and causes multisystemic changes. An acute exacerbation of hyperthyroidism is called thyrotoxicosis, and is a medical emergency that may lead to cardiac failure. Cause: Believe to be an autoimmune disease. Antibodies to thyroglobulin or to thyroid microsomes occur in most patients with this disorder. Symptoms: Enlarged thyroid

Goiter Goiters can be any one of several types of growths in the thyroid gland, located at the base of the neck. In the case of Graves' disease , the entire thyroid gland becomes enlarged. Another type, called nodular goiter , results when one or more nodules, or adenomas, develop in the thyroid and trigger excess production of thyroid hormone. Goiter refers to any visible enlargement of the thyroid gland.
Description
The thyroid gland sits astride the trachea (windpipe) and is shaped like a butterfly. It makes thyroxin, a hormone that regulates the metabolic activity of the body, rather like the gas pedal on a car. Too much thyroxin increases the metabolism, causing weight loss, temperature elevation, nervousness, and irritability. Too little thyroxin slows the metabolism down, deepens the voice, causes weight gain and water retention, and retards growth and mental development in children. Both conditions also alter hair and skin growth, bowel function, and menstrual flow. Curiously, the thyroid gland is often enlarged whether it is making too much hormone, too little, or sometimes even when it is functioning normally. The thyroid is controlled by the pituitary gland, which secretes thyroid stimulating hormone (TSH) in response to the amount of thyroxin it finds in the blood. TSH increases the amount of thyroxin secreted by the thyroid and also causes the thyroid gland to grow.

When dietary iodine is inadequate, the thyroid gland swells. Swelling is a result of follicular epithelial cell hypertrophy, hyperplasia or both. This produces a bulge at the base of the neck which can become quite pronounced. I have seen women tie a scarf around their goiter to support its weight. A goiter can compress the trachea and cause a hoarse voice, chronic coughing, difficulty in swallowing and facial congestion. My mother's goiter grew down inside her ribs and caused problems swallowing. Iodine supplements can be used for the prevention and cure of simple goiter. The Chinese recorded the use of seaweed, which contains iodine, for goiter since about 3,000 B.C. 96% of goiters are caused by an iodine deficiency. Even when there is not enough iodine available, thyroid hormone secretion continues until available iodine is exhausted. The pituitary continues to release Thyroid Stimulating Hormone (TSH) which causes thyroid activity to keep increasing. This stimulates thyroid gland tissue enlargement and growth (goiter). Over 200,000,000 people worldwide have developed goiters. Because of iodized salt, goiters are increasingly rare in industrialized countries. The Great Lakes area of the United States has a low water concentration of iodine and is well-known for its high incidence of goiter. (see "Iodized salt" below)

Possible Causes
  • Iodine deficiency is the largest known cause of goiter. Iodine is vital to the formation of thyroid hormone.
  • An overproduction of thyroid stimulating hormone in the pituitary gland in response to a defect in normal hormone synthesis within the thyroid gland.
  • Certain medications in high doses, such as lithium or aminoglutethimide can cause a nontoxic goiter.
  • A family history of congenital goiter.
  • In most cases of sporadic goiter the cause is unknown.

Possible Complications
  • Both under and overactive thyroid, if left untreated, can lead to life-threatening complications.
  • The goiter mass will sometimes compress the trachea (windpipe) and esophagus (swallowing tube), causing difficulty in breathing and swallowing.
  • Malignancy (cancer) within a multinodular enlarged thyroid is seldom the case, but does happen is at least 5% of cases.
  • Thyrotoxicosis, an excessive amount of thyroid hormone in the bloodstream.

  • Goiter is an enlargement of the thyroid gland. The thyroid gland is located in front of and on the sides of the "Adam's apple" on the neck. The cause of endemic goiter is iodine deficiency in the diet. Thus the term endemic refers to goiter that is prevalent in a certain part or region of the world, usually mountain areas where the soil is low in iodine content. Less commonly, goiter is caused by substances in the food or water that increase the likelihood of iodine deficiency.

Hyperthyroid disorders don't always require the same treatment that I had.
Multinodular goiter is treated differently than Graves' Disease - I had no medication options. Molly b.
December, 2000 May 1997: I went to a doctor about feeling faint, dry eyes and weight loss. A series of blood tests showed that I was hyperthyroid. An appointment with a specialist was scheduled for October. I was given a prescription for Ativan, to take as needed, for 'anxiety attacks' which were really just rapid heartbeat reactions caused by the hyperthyroid symptoms. Five months is a very long time to be hyperthyroid with no treatment. Long term untreated hyperthyroidism can result in heart damage and more. If you have a hyperthyroid condition, don't delay getting it treated. I wouldn't have waited 5 months, if I'd had a choice. (This is what health 'cutbacks' do.)
