  • erythrovirus (formerly parvovirus) type B19 is the cause of 'fifth disease' (also known as slapped cheek disease and erythema infectiosum) infection with erythrovirus B19 is uncommon in pregnancy there is a risk of fetal death in about 10% of cases. Adverse fetal/neonatal outcomes associated with fetal infection include:
      spontaneous abortion (occurs in about 10% affected before 20 weeks gestation), stillbirth, and non-immune hydrops foetalis (if severe then this is treated by intrauterine transfusion), growth retardation, myocarditis/infarction, meconium peritonitis, placentomegaly, oedema, anaemia, rashes, thrombocytopaenia, leucopenia, respiratory insufficiency
    fetal risk is greatest in the second trimester there is an approximate 1% risk of congenital abnormality (1)

(Parvovirus B19) What is Erythema infectiosum? (F ifth disease)
Erythema infectiosum
is a mild illness caused by Parvovirus B19. It is usually non-febrile. Severe complications resulting from this illness are rare although persons with chronic blood disorders who also get fifth's disease may develop an inability to form new blood cells (Erythrocytes). Women who are exposed to the disease during pregnancy are at risk as the virus can infect the unborn child, which may result in death of the fetus. How do you get it?
The virus spreads by exposure to secretions or droplets from the nose and throat of persons with the disease. Fifth's disease occurs most commonly in children of elementary school age but anyone can get be susceptible.
Pregnant women and persons with red blood cell disorders who have been exposed to Parvovirus B19 should consult with their doctor. What are the symptoms of fifth disease?
Fifth disease initially starts with a low grade fever and fatigue. A striking red rash (which looks much like a slap mark) may appear on the cheeks. A fine (lace-like) rash may then appear on the body. Sometimes the rash may be itchy. Some children may have mild signs of illness or no symptoms at all. When do symptoms start?

  • Fifth disease is caused by a viral infection called Parvovirus B19.
Who can get fifth disease?
  • Anyone can get fifth disease in any part of the world. It is most common in children ages 5-15. Infections happen most often in the late winter and early spring.
What are the symptoms of fifth disease?
  • Symptoms may begin like a mild cold, with a slight fever, stuffy or runny nose, and a headache. These symptoms usually pass. A rash may show up 6-11 days later. The bright red rash usually begins on the face. The cheeks look like they have been slapped. The rash can spread in blotches to the trunk, arms, legs, and bottom. The center of the rash may clear up. The rash looks like a net or lace.

(Erythema nodosum) — îäíî èç ïðîÿâëåíèé ðÿäà èíôåêöèîííûõ áîëåçíåé (òóëÿðåìèÿ, ðåâìàòèçì, òóáåðêóëåç è äð.). Õàðàêòåðèçóåòñÿ ïîâûøåíèåì òåìïåðàòóðû òåëà (åñëè îíà íå áûëà óæå ïîâûøåíà çà ñ÷åò îñíîâíîãî çàáîëåâàíèÿ), ëîìÿùèìè áîëÿìè â êîíå÷íîñòÿõ, îñîáåííî â êðóïíûõ ñóñòàâàõ, è õàðàêòåðíîé ýêçàíòåìîé. Ñûïü ïîÿâëÿåòñÿ íà ñèììåòðè÷íûõ ó÷àñòêàõ ãîëåíåé è ïðåäïëå÷èé, ðåæå íà áåäðàõ è ñòîïàõ. Ýëåìåíòû ñûïè ïðåäñòàâëÿþò ñîáîé óçëû (äî 3—5 ñì â äèàìåòðå), ïëîòíûå è áîëåçíåííûå ïðè ïàëüïàöèè, âîçâûøàþùèåñÿ íàä óðîâíåì êîæè, â ãëóáèíå êîæè ïðîùóïûâàåòñÿ èíôèëüòðàò. Îêðàñêà êîæè íàä óçëàìè ñíà÷àëà êðàñíàÿ, çàòåì ñòàíîâèòñÿ öèàíîòè÷íîé è ïðè óìåíüøåíèè èíôèëüòðàòà — çåëåíîâàòî-æåëòîâàòîé. Óçëû ñîõðàíÿþòñÿ äî 3 íåä.
(Erythema åõèdativa multiforme) õàðàêòåðèçóåòñÿ ëèõîðàäêîé (39—40°Ñ), ñèìïòîìàìè îáùåé èíòîêñèêàöèè (ãîëîâíàÿ áîëü, ñëàáîñòü, àðòðàëãèÿ). Íà 4—6-é äåíü ïîÿâëÿåòñÿ îáèëüíàÿ ïîëèìîðôíàÿ ñûïü, çàõâàòûâàþùàÿ òóëîâèùå è êîíå÷íîñòè. Èíîãäà îòìå÷àåòñÿ ñèììåòðè÷íîñòü âûñûïàíèé. Ýëåìåíòû ñûïè ïðåäñòàâëÿþò ñîáîé ïÿòíà, ïàïóëû. Îñîáåííî õàðàêòåðíî îáðàçîâàíèå ïóçûðåé, íàïîëíåííûõ ïðîçðà÷íûì ñîäåðæèìûì. Ïîñëå ðàçðûâà ïóçûðÿ íà åãî ìåñòå îáðàçóþòñÿ âíà÷àëå êðàñíàÿ ññàäèíà, à çàòåì áóðîâàòàÿ êîðî÷êà. Íà ìåñòå âûñûïàíèÿ áîëüíûå îòìå÷àþò çóä è ææåíèå êîæè. Ïðè áîëåå òÿæåëîì âàðèàíòå ýðèòåìû — ñèíäðîìå Ñòèâåíñà—Äæîíñîíà, ïîìèìî ïîðàæåíèÿ êîæè âîçíèêàþò ýðîçèâíî-ÿçâåííûå èçìåíåíèÿ ñëèçèñòîé îáîëî÷êè ðòà, íîñîãëîòêè, ïîëîâûõ îðãàíîâ, àíóñà. Áîëåçíü äëèòñÿ îáû÷íî 1~3 íåä, à ñèíäðîì Ñòèâåíñà—Äæîíñîíà — äî 6 íåä è áîëåå. Îïèñàíû ëåòàëüíûå èñõîäû.
