Epididymitis The epididymis is a structure of tubules that transport sperm from the testicle. It is palpable on physical examination on the posterior or rear of the testicle. The tubules within the epididymis merge to form the vas deferens. The vas deferens is the tube that is cut during a vasectomy to prevent sperm from mixing with the remainder of ejaculatory fluid.

EPIDIDYMITIS The epididymis is a structure which lies on and around each testicle. It functions in the transport, storage and maturation of sperm cells originating from the testicle. When a man complains of scrotal pain, acute or chronic epididymitis is far and away that most common diagnosis. Acute epididymitis is usually more severe involving more significant swelling and pain than chronic epididymitis. Epididymitis which lasts more than six weeks is considered chronic epididymtitis. Symptoms and Causes The symptoms of epididymitis include scrotal pain and swelling. The pain or swelling may be mild or severe. At times the epididymus may become so inflamed that a patient is unable to walk (due to pain). Infection may also become so severe that it spreads to the adjacent testicle. Such cases may cause fever and, rarely, abscess formation. Most cases of epididymitis are not severely disabling; usually the pain and swelling is not severe. It is common for pain to increase as the day progresses. It has long been thought that straining or strainful work was a contributory factor in epididymitis. Recently, however, this has been considered to be less of a factor and infection is now considered the most common cause of epididymitis. The infective agents most likely to cause epididymitis are the bacteria which commonly cause other urinary infections. A much less common cause of epididymitis is the gonorrhea bacteria. Treatment The treatment for acute epididymitis is well accepted and effective. It includes antibiotics therapy, bed rest, scrotal support ("supporter"), and oral anti-inflammatory drugs (such as Ibuprofen). Each of these modes of treatment is important. Chronic epididymitis is more of a problem (though less severe) because it's symptoms seem to persist even after the initial treatment. In these cases a second round of therapy may be helpful. Beyond this, longer term anti-inflammatory medication is recommended. Surgical treatment for chronic epididymitis is an uncommon last resort.

There are not different strains of one bacteria causing this disease, but two different bacteria causing almost identical symptoms in rams. In western Canada the principle bacteria implicated are Brucella ovis and Actinobacillus seminis . In Ontario there have been very few isolations of B. ovis , but epididymitis is still a problem. In this province the bacteria thought to be the cause is Actinobacillus seminis . However, the result is the same, a chronic infection of the epididymis and testes with lowered fertility of the ram. There is no treatment for these conditions. Ram management is important. Young rams should be run separate to older rams, and wherever possible only used to breed first lamb ewes. In some countries vaccines are used to protect against B. ovis with some success, but with unwanted side-effects. These vaccines are not available in Canada. Top of Page

Although the infection can affect both sides of the epididymis, it most often occurs on one side only. SIGNS OF EPIDIDYMITIS When the scrotum gradually becomes painful over the course of several hours or days, epididymitis is likely. The pain is generally severe and is accompanied by fever, swelling and tenderness. CAUSES A bacterial infection which originates in the bladder or prostate is most often the cause of epididymitis. The infection may also be caused by a sexually transmitted disease such as gonorrhea or chlamydia. In older men, the common cause is coliform bacteria, which are found in the gastrointestinal tract and usually enter through the urethra. TREATMENT Your doctor will generally prescribe antibiotics. You sex partner will sometimes need to be treated at the same time to prevent reinfection. Bed rest and application of ice packs will help relieve the pain. Also elevating the scrotum with a rolled up towel for support may relieve discomfort. Sexual activity should be suspended until the infection clears up. An athletic supporter can be worn to ease discomfort during recuperation.
TIPS FOR PREVENTION
Keep the area of the penis and scrotum clean.

If not treated, or in some other cases, the condition can become chronic. In chronic cases, there is usually no swelling, but simply pain. The incidence is approximately 600,000 cases per year. The highest prevalence is in young men 19 to 35 years of age. The disorder is a major cause of hospital admissions in the military (causing approximately 20% of admissions). Epididymitis is caused by spread of infection from the urethra or the bladder. The most common organisms involved in the condition in young heterosexual men are gonorrhoea and chlamydia.
