    The role of vaginal apex excision in the management of persistent posthysterectomy dyspareunia
    Am J Obstet Gynecol, Dec. 2000; 183: 1385-9
    Objective: The purpose of this study was to evaluate the effectiveness of vaginal apex excision in the treatment of patients with posthysterectomy dyspareunia.
    Study Design : This was a case series with an independent third-party survey of patients with posthysterectomy dyspareunia managed at the University of Utah Pelvic Pain Clinic. Thirteen patients were first treated with local injections of anaesthetics into localized vaginal pain foci. Further evaluation included formal psychometric testing and a diagnostic spinal block. Nine patients underwent surgical excision of the vaginal apex. An independent interviewer who did not know the patients assessed the effects of this procedure on dyspareunia and coital frequency at a mean of 36.4 ± 3.7 months after the operation.
    Conclusion: Excision of the vaginal apex is an effective treatment for carefully selected patients with posthysterectomy dyspareunia.

Recurrent and persistent pain associated with sexual intercourse. FREQUENT SIGNS AND SYMPTOMS
Pain in the genital area during sexual activity, including foreplay, intercourse or attempted intercourse. Pain may be mild or severe, and it may vary with different intercourse positions. CAUSES
Physical causes include:
Infection of the genitals, including herpes and others involving the vagina, cervix, fallopian tubes or ovaries.
Pressure against the vaginal wall caused by scarring from operations or radiation treatment.
A tight episiotomy scar from vaginal repair after childbirth.
A fibroid or other uterine tumor.
Endometriosis
A hymen that is torn or thicker than normal. A bruised opening to the urethra. Inadequate vaginal or condom lubrication. Allergic reactions to diaphragms, condoms or contraceptive foams and jellies. Dryness and thinness of the vaginal wall after menopause. Psychological and emotional causes include: Fear of pregnancy. Fear of injury to the unborn child during pregnancy. Lack of sexual arousal and vaginal lubrication caused by inadequate or insufficient sexual foreplay, aversion to sexual partner, fatigue or anxiety.

Dyspareunia is pain in the vagina when the penis is inserted during sexual intercourse.
What is going on in the body? Pain during sexual intercourse can be upsetting. The cause of the pain may be hard to find. There might be physical and psychological causes. Physical causes may be within the female reproductive tract, including the vulva, vagina, cervix, uterus, Fallopian tubes, ovaries and abdominal cavity. Some women may assume that sex will be painful. Or they might associate menstruation and childbirth with pain. Even when no physical causes are found and the pain is gone, the memory of the pain may persist. This can interfere with pleasure. The range of physical and psychological factors that prevent a woman from enjoying a sexual relationship should be considered. What are the signs and symptoms of the condition?

Q 23. What causes pain associated with sexual intercourse (dyspareunia)? Dyspareunia is a very distressing problem, which may have dire effects on a relationship. Two types of dyspareunia relating to their site are recognised. Superficial dyspareunia is pain around the entrance to the vagina. Superficial dyspareunia may be due to vulval problems ( Q 31.1 ), vestibulitis ( Q 31.13 ) or vaginal problems such as vaginitis ( Q 22.6 ). Another possible cause is vaginismus where there is tension or spasm of the muscles around the vaginal entrance. The spasm may be such that the couple believe that there is an obstruction requiring surgical correction. Sometimes there is deeper pain and vaginismus is a protective mechanism. More frequently there is no disease but a functional problem due to an earlier sexual encounter, fear of pregnancy or inadequate arousal. Deep dyspareunia is pain deep inside the vagina and pelvis. As the reigning Queen of "Not Tonight, Honey...," I have somewhat learned how to deal with this unfortunate by-product of Endometriosis. Make no mistake: Endometriosis is a disease that affects more than just the woman. It affects her partner almost as much, and dealing with it as a couple is crucial. Let's first address why some patients with Endometriosis experience dyspareunia (painful intercourse): the lesions may obstruct, bind or invade tissues and organs; it can secrete inflammatory substances known as prostaglandins and histamines that irritate surrounding tissue; it can cause scarring which can restrict blood vessels; and it can make our pain receptors much more sensitive. Cul-de-sac (the membranous wall between the rectum and the vagina) Endo is a known culprit for causing painful intercourse, particularly upon deep penetration. Take these physical aspects of the disease and combine them with treatment medications that kill our libido, throw our hormones into overdrive and in some cases, make us gain weight so we can have a whole new set of body-concious issues to deal with, and you've got a recipe guaranteed to have "Not Tonight, Honey..." coming soon to a bedroom near you. Further, let us not forget that infertility also factors into the mix for some. Dyspareunia Dyspareunia is the clinical name for painful intercourse. This condition can occur at any age, in both sexes, and the pain can appear at the start of intercourse, midway through, at the time of orgasm, or after intercourse is completed. The pain can be felt as burning, sharp, searing or cramping. It can be external, within the vagina, or deep in the pelvic region or abdomen. The causes of dyspareunia, as with most sexual dysfunction, can be classified as either organic (physical or medical factors such as illness, injury or drug effects) or psychosocial (including psychological, interpersonal, environmental and cultural factors). The cause of a sexual dysfunction in a given individual may be a combination of several factors, and in some cases, the precise cause may not be identifiable at all. Other feelings and psychological conflicts may also affect sexual responsiveness, inhibiting or reducing vaginal lubrication, which can result in painful intercourse. Guilt, depression and poor self-esteem are commonly encountered in association with sexual dysfunctions. However, it is not always clear which came first, the feelings or the dysfunction. Because it is not unusual for people who have sexual problems to become depressed about them or to experience lowered self-esteem, identifying a problematic feeling does not always mean it caused the dysfunction. Dyspareunia is generally thought of as a female sexual dysfunction but it also affects males. Typically, the pain is felt in the penis but it can also be felt in the testes or internally, where it is often associated with a problem of the prostate or seminal vesicles.

Considerations: It is believed that dyspareunia is caused by organic (physiological) factors at least 75% to 80% of the time. Pain that occurs initially at entry but decreases over time is commonly caused by inadequate lubrication.
