This break in the anal lining often reopens during bowel movements. The result is bright red blood and intense pain. What is going on in the body? An anal fissure is a tear in the bowel lining. It causes painful, bloody bowel movements. It is often confused with hemorrhoids, which are painful swellings at the anus caused by enlarged veins. A fissure can be acute or chronic. The chronic condition is often associated with a buildup of tissue at the external end of the tear. This is called a sentinel pile and may be noticed by an affected person. The internal end may also have a buildup of tissue that an affected person can rarely see. When a fissure is present, the mucosa of the anus opens each time it is stretched to allow for a bowel movement. This continual opening prevents healing. It can also leave scar tissue. Most chronic fissures are in the center of the back of the anus. The anal sphincter muscles need to relax for a person to have a bowel movement. A person with a fissure may have muscles that are too tight. This makes bowel movements painful.

Fissures begin as a small tear or ulceration, due to either a bout of diarrhoea or sometimes a particularly large bowel movement. Often, the anus is already chronically irritated by something else. The vast majority of fissures resolve entirely within a day or so and rarely cause any prolonged problem. Symptoms are sharp pain and a small amount of bright blood during a bowel movement The symptoms of fissure are sharp, knife-like pain in the anus during a bowel movement, and often the presence of a small amount of bright red blood streaking the toilet paper during wiping. Fissures rarely bleed more than a small amount. Sometimes, if a fissure becomes chronic (long-standing) and deep, another kind of pain can develop. It is a dull ache in the anus and pelvis that starts a few minutes after the bowel movement and can persist for an hour or two before gradually subsiding. This chronic fissure pain is due to spasm of the internal sphincter muscle (one of the small muscles in the anus) and can be quite debilitating. It is like a recurring "Charley Horse" of the anus and can be so bad that patients can no longer work.

Feedback DEFINITION Anal fissures are shallow, thin, often painful ulcers of the anal canal. CAUSE Anal fissure can result from passage of large, hard stools or may be a secondary result of anal surgery, proctitis, or other diseases. Since the surface of the skin at the anus has been worn away, defecation irritates the fissure and may cause the sphincter muscles to spasm, resulting in intense pain. Fissures are often associated with constipation or inadequately emptied stools, and may also be linked to stress or hemorrhoids. DIAGNOSIS During a physical examination to locate the source of pain, the fissure can usually be seen, although anoscopy, in which a small plastic or metal instrument called an anoscope is inserted into the lower few inches of the rectum, may be used to confirm the findings. Flexible endoscopy is also useful in establishing a diagnosis. TREATMENT Anal fissures are treated with warm baths and topical creams; stool softeners (brans such as psyllium) are added to the diet. Surgery is generally unnecessary but may be recommended in severe cases. After the fissure heals—which may take 2 to 3 weeks—a sigmoidoscopy may be performed to check for other rectal problems.

Anal fissures may be associated with secondary changes such as a sentinel tag, hypertrophied anal papilla, induration of the edge of the fissure, and anal stenosis. A patient with multiple fissures, or whose fissure is not in the midline, is more likely to have Crohn's disease. There may be blood on the outside of the stool or on the toilet tissue following a bowel movement. Causes, incidence, and risk factors Return to top Anal fissures are extremely common in young infants but may occur at any age. Studies suggest 80% of infants will have had an anal fissure by age one. Most fissures heal on their own and do not require treatment, aside from good diaper hygeine. However, some fissures may require medical treatment. The incidence of anal fissures decreases rapidly with age. Fissures are much less common among school-aged children than among infants. In adults, fissures may be caused by constipation , particularly when passing large, hard stools, or by prolonged diarrhea.
