Superficial Thrombophlebitis Superficial thrombophlebitis is an inflammation of a 'surface' vein, usually on a leg. A small blood clot may form in the vein, but is usually not serious. The condition usually settles and goes within 2-6 weeks. Treatments can ease the pain and discomfort. Complications occur in a small number of cases. What is thrombophlebitis? Phlebitis means inflammation of a vein. Thrombo(sis) means a blood clot in a vein A vein is a blood vessel that takes blood towards the heart. If a vein becomes inflamed, a blood clot commonly forms inside the inflamed portion. So, the term thrombophlebitis is used to mean an inflamed vein, with or without a small blood clot inside the vein. (However, thrombophlebitis is commonly just called phlebitis.) What is superficial thrombophlebitis? The superficial veins are the ones that you can often see or feel just under the skin. Most bouts of thrombophlebitis occur in a leg vein, but any superficial vein can be affected. A typical site is in a varicose vein in a leg. Varicose veins are common, particularly in pregnant women. Superficial thrombophlebitis is not usually serious, but complications sometimes occur (see below). Note: this leaflet does not deal with inflammation or thrombosis of deep veins which are larger, pass through the muscles in your arms and legs, and you cannot see or feel. People often get confused between thrombophlebitis and deep vein thrombosis, but they are quite different. A deep vein thrombosis (DVT) is more serious, and is dealt with in another leaflet.

The complications that can occur may be serious, so it is important to know a bit about the condition. To understand phlebitis, it is helpful to remember that arteries bring the blood from the heart into the head, arms, and legs and end in the network of small capillaries which then pass on into the veins that take the blood back to the heart and lungs. Blood flows more slowly through the veins and especially in varicose veins where they become dilated and more torturous. When added pressure is put on the veins by a lot of standing and squatting, by crossing the legs and by getting bumped, an inflammation takes place. This causes the area to become swollen, painful, red, and tender to touch. Once it begins, it can extend up and down the leg and may cause fever. At this point, it becomes urgent to be checked by the doctor and start immediate treatment. It is at this time that blood clots can develop and, if dislodged, circulate into the heart or lungs with fatal results.

Thrombosis is the formation of a blood clot that may partially or completely block a blood vessel. A clot located in an inflamed, blood vessel is called thrombophlebitis. A clot formation in a deeper vein is called a deep vein thrombosis. Blood clots may follow injury, surgery, childbirth, or prolonged bed rest. They may also be associated with infection. Currently she is on Methotrexate, Ticlid(since a stroke in '96) and cortisone, as well as buffered aspirin. Last month she also started taking estrogen. She was recently diagnosed with phlebitis in her right arm(not the stroke affected side). The doctor ran blood tests and found that her LDH (is that right?) was elevated, and also noted that the phlebitis seemed to be migrating from place to place in her arm. He said this is sometimes seen in cancer patients, and mentioned the possibility of tumors in the pancreas or liver. Is this a liklihood, or just a possibility that needs to be ruled out? Is there something else that might be causing these problems? She has since stopped taking the estrogen, and is scheduled for an abdominal and pelvic CAT scan. We are very frightened. Her legs are very swollen, hot and painful. Would you explain phlebitis and why it is painful? Answer: Phlebitis, which is also called thrombophlebitis, is the inflammation of a vein and should not be confused with inflammation of an artery. The condition can be divided into two broad categories based upon the vein or veins that are involved superficial and deep. Superficial veins are those that run close to the skin while deep veins are those that are within muscles. Leg swelling can result from phlebitis of either the superficial or deep veins, so I'll start by explaining the superficial variety first. Superficial veins, usually those of the arms or legs, can become inflamed from injury. The most common causes of this are trauma from an accident or from medical treatments such as receiving an IV or an injection of medicine into the vein. If you develop this type of phlebitis, the first thing you'll probably notice is a red and swollen area over the vein. Later, a blood clot will form within the vein and, thereby, block the flow of blood. This causes some additional swelling and pain at the site of the phlebitis. Though superficial phlebitis is painful, it isn't a life threatening condition because there are many other superficial veins that can assume the work of the damaged one. Also, the risk of the blood clot breaking loose and causing serious problems is quite small.

When the affected vein is close to the surface, the condition is called superficial phlebitis. This condition usually resolves on its own without further complications. However, when phlebitis occurs in a deep vein, it is called deep vein thrombosis (DVT). DVT is a life-threatening condition because a clot could dislodge from the vein and lodge in the lungs. If a swollen, painful vein does not disappear within 2 weeks, consult a physician. Symptoms of superficial phlebitis include pain, swelling, redness, and warmth around the affected vein.
