Empieza a escribir durante la dictadura de Porfirio Díaz: María Luisa (1907), su primera novela, la publicó cuando era estudiante; visionario, advierte en Andrés Pérez, maderista (1911) los cambios revolucionarios incipientes; se integra como médico a las fuerzas villistas durante la Revolución Mexicana y escribe Los de abajo (1915), la novela que lo haría famoso, publicada primero como folletín en Texas: "novela de ese primer momento de la Revolución Mexicana en que principia la lucha con una cólera ciega" según Castro Leal, inaugura un estilo nuevo acorde con la lucha armada en la que destacan los cuadros rápidos, violentos, realistas. Con La malhora El desquite La Luciérnaga (1932) inicia un periodo de experimentación vanguardista. In 1915 he joined Francisco Villa's revolutionary forces as a surgeon. From this experience came his modern classic, Los de abajo (1915, tr. The Underdogs, 1929), which depicts the military exploitation of indigenous people. The novel is composed of linked sketches that are starkly realistic. After Villa's defeat Azuela took refuge in Texas. Returning to Mexico in 1916, he resumed his medical practice and his writing, taking little interest in politics. Among his later novels are Los fracasados [the defeated] (1908); Mala yerba Los caciques (1917, tr. The Bosses, Las moscas (1918, tr. The Flies, 1956); and San Gabriel de Valdivias

Azuela nieto dijo que la obra, publicada por Plaza y Valdés, responde a un deseo de desmitificar a "San" Mariano Azuela, un personaje que conoció cuando era un niño y al que siempre vio como "un hombre de carne y hueso". "La crítica literaria y un grupo de mis familiares hicieron de Mariano Azuela una especie de santón de las letras y un hombre inmaculado". Para hacer frente a ese "proceso de canonización", Arturo Azuela traza en "Prisma" un retrato humano de un personaje con una biografía apasionante que discurrió en paralelo a los años más crudos y trascendentales de la historia de México.
