Editorial Review Product Description Organ transplantation has been performed for almost 40 years with steadily increasing success regarding long-time survival of the graft, as well as quality of life for the patient. However, graft tolerance only can be achieved via induction and maintenance of an impaired immunological surveillance. An increase of skin cancers as a consequence of the lowered cellular immunoresponse seems to parallel the overall increased long-time survival rate of organ transplant recipients. Against the background of chronic immunosuppression known risk factors like the amount of sun exposure prior and post transplantation, oncogenic viruses as well as the genetic background, and place of residence (latitude) are strongly related with the increased skin cancer incidence. The increasing incidence of non-melanoma skin cancer, paralleling a prolonged survival of patients after organ transplantation, represents a significant reason for morbidity and long-term morbidity in organ transplant recipients worldwide. The incidence of non-melanoma skin cancer in liver-, kidney- and heart transplant recipients varies from 1.5 to 22 %, 2 to 24 % and 6 to 34 % after < 5 years post transplant. Ultraviolet radiation as well as immunosuppressive therapy are crucial risk-factors regarding the induction and progression of skin cancer. Ultraviolet radiation is related to the induction of DNA damage, as well as interference with Langerhans cell antigen presentation and a Th1 – Th2 shift induced via a release of IL-10. Whereas the overall duration of immunosuppression and the cumulative dosage applied are relevant parameters in the evaluation of an increased tumor risk, individual differences between specific immunosuppressive agents remain unclear. The workup of genetic as well as other unclear phenomenon like the reverse BCC/SCC ratio might shed some further light into the genesis and immunology of the non-melanoma skin cancer in general. |

The book offers guidelines on how and when to biopsy the skin and then discusses benign melanocytic proliferations, precursor lesions to melanoma, and melanomas. Also included are discussions of basal cell carcinoma, squamous cell carcinoma and its precursors, benign and malignant adnexal neoplasms, and soft tissue neoplasms of the skin and superficial subcutis. A major highlight of the book is the extensive discussion of differential diagnostic considerations and their separation from the main diagnostic entities under consideration. The book features nearly 300 full-color images. A companion Website will provide 400 additional figures, plus a test bank that is ideal for board exam preparation. |

Editorial Review Product Description This book, written by the founders of Coolibar, the only provider of solar-protective clothing endorsed by the Skin Cancer Foundation, tells readers everything they need to know to protect themselves from the sun, prevent life-threatening skin cancer, and preserve their skin's natural youth and beauty. It features expert advice from doctors at the Mayo Clinic, the Skin Cancer Foundation, The American Academy of Dermatology, and the Environmental Protection Agency. The book prescribes a lifestyle that allows readers to enjoy the sun, while protecting themselves from its harmful rays. It explains the important things to look for in sunscreens, protective clothing, and sunglasses. It explains to readers how and when to go into the sun, and what regular medical check-ups they should schedule to help prevent undetected skin cancer. For parents, the book offers many tips on protecting kids from the sun. In addition to discussions about the changing environmental factors that influence solar exposure, the book includes information about sun protection education programs and recently enacted solar protection policies. The book has a slightly doomed tone, but perhaps that's as it should be; malignant melanoma is deadly, and statistics chart a steady rise in incidence, particularly as tan-loving baby boomers continue ageing. As the authors are founders of Coolibar sun-protective clothing, one would normally expect some degree of product placement, but that's not the case in this book. There is a lengthy chapter on what degree of protection clothing provides, as well as what to look for when purchasing summer wear--but there are equally detailed descriptions of types of sunscreen, suggestions for different age brackets, and information on identifying potentially cancerous growths. The book ends with a resource guide, covering everything from international skin care sources to, yes, sun-protective clothing companies. What's most impressive is the way in-depth detail is presented with such a matter-of-fact manner; it's easily understandable for anyone past the ninth grade. It also offers steady encouragement for turning the dangerous desire for a tan into a healthy respect for the sun, with regular examples from Australia's skin cancer awareness program that has made incredibly positive changes in that country's cancer rates. While you might not feel the need to join a nation crusade on the topic, at the very least you'll be grabbing a cap and a tube of zinc oxide before leaving home. Jill Lightner Customer Reviews (2)
