Coping Thirteen years ago, I miscarried the baby my husband and I had struggled so hard to conceive, an experience I wrote about for the August 1988 issue of Parents' Press. Two years and two more miscarriages later, I managed yet another pregnancy, aided and encouraged by my husband and a support group, and some mild medical intervention. This time we were more fortunate: our daughter Margaret Isobel, a vigorous nine-pounder, came wailing into the world on Aug. 22, 1989. Again, I reported back to Parents' Press about the highs and lows of parenting after infertility. The low was postpartum depression. What follows here is a revised version of my 1991 account, with resource updates. The pregnancy was physically easy, but labor was hard: 24 erratic hours, followed by three hours of fruitless pushing before we decided that our obstetrician was right: there was no way Margaret would emerge except through my abdomen. I was disappointed to have gone so long only to end with a C-section, but exhilarated to finally stop hurting and to see our little girl.

How can we help new moms avoid this problem? News 5 Medical Team reporter Dr. Walt Larimore explains. About 10 to 15 percent of new mothers experience post-partum depression. It can range from mild feelings of loneliness to severe psychotic episodes. We may be able to learn something from a new British study about how to combat post-partum depression. In the study, British woman, after giving birth, were either visited by a midwife six to seven times during the two weeks after delivery, or received normal care, seeing her doctor after six to eight weeks. Researchers studied about 2,000 women who received either standard care, or had additional visits by midwives 10 to 12 weeks after birth. The women who received the extra visits had better overall mental health. Children of mothers with post-partum depression can have increased problems with development and behavior, so preventing the problem is important for the children too.

Omega-3 fatty acids are found in cell membranes throughout the body, including those in the brain's nerve cells. Over the years, changes in the American diet have lead to a decrease in dietary Omega-3, found in cold-water fish such as cod, tuna, salmon and sardines. "While Omega-3 has decreased, Omega-6 fatty acids, also found in the cell membranes, have increased," said Dr. Lucy Puryear, director of The Women's Clinic in the Department of Psychiatry at Baylor College of Medicine in Houston. "We think this shift is affecting brain chemical receptors and impacting the brain's levels of serotonin, which is thought to be linked to depression." Studies have shown that people with depression have lower levels of Omega-3. "We also know that during pregnancy the fetus is taking Omega-3 from the mother to make brain and nervous tissue," Puryear said. Women with post-partum depression experience sleep difficulties, excessive crying, loss of energy and appetite, and a loss of interest in activities. The condition generally begins within the first four weeks but can occur up to four-to-six months after delivery.

DHA supplements may also reduce the incidence of postpartum depression. "We believe that the high incidence of postpartum depression in the United States may be triggered by a low dietary intake of DHA," presenter David J. Kyle, PhD, from the Mother and Child Foundation and Advanced BioNutrition Corp in Columbia, Maryland, said in a news release. "The higher the intake of DHA, the lower the incidence of depression." A 1998 study by Joseph Hibbeln of the National Institutes of Health found a significant inverse correlation between DHA intake and incidence of clinical depression, and a more recent study by Hibbeln found the same relationship between DHA levels in breast milk and incidence of postpartum depression. During pregnancy, the placenta pumps DHA from the expectant mother to the fetus, increasing the mother's susceptibility to depression.

These same statistics also record that suicide is the most common form of maternal death in the first year after the birth. As with any form of Depressive illness, Post-natal depression can be so severe, it may require hospitalisation. Post-natal depression conforms to all of the symptoms of a typical depressive episode (persistent low mood, appetite loss irritability physical and mental fatigue etc), and can be, mild, moderate or severe. The fundamental difference between these blues and Post-natal depression is that, women with PND do not bounce back. The baby blues typically resolves within about ten days after delivery. Post partum depression lingers. The baby blues cause mild sadness and mood swings. PPD causes severe depression and loss of interest in life. After pregnancy, progesterone levels fall drastically. Like progesterone, the levels of estrogen, a hormone that seems to be important in memory and mood, fall 90 to 95 percent in the first 48 hours following delivery. Thyroid levels may also drop sharply after birth. A thyroid deficiency can produce symptoms that mimic depression, such as mood swings, severe agitation, fatigue, insomnia, and anxiety. A link between estrogen levels and postpartum depression has not been established [McEwen, 1993] and experts cannot agree on the role that hormone fluctuations play in post partum depression.
