Streptococcus pneumoniae remains the leading cause of bacterial meningitis. While a new vaccine against this organism is now available, its impact on disease is not yet known. Strategies to guide the evaluation of patients and the treatment of antibiotic-resistant strains can help to improve outcomes in this dangerous disease.

Despite diagnostic and therapeutic advances, the outcome for patients with community-acquired bacterial meningitis is often unfavorable. Streptococcus pneumoniae is currently the most common cause of bacterial meningitis in both children and adults in the United States and Canada. Unfortunately, the estimated mortality from pneumococcal meningitis is approximately 20%, higher than for any other bacterial etiology.

Bench research has helped to elucidate the pathogenesis and pathophysiology of pneumococcal meningitis and may lead to new therapeutic strategies. Clinical research efforts have also been fruitful, helping to resolve management-related controversies, including the role of screening head computed tomography (CT) scans, the impact of antibiotic timing on outcome, and the choice of empiric antibiotics in this era of penicillin resistance.

In the preantibiotic era, virtually all patients with bacterial meningitis died. The introduction of antibiotics brought a substantial reduction in the number of fatal cases. In the 21st century, approximately 25% of adults with bacterial meningitis still die of their disease, and at least 10% are left with a persistent neurologic deficit.

Over the past decade, two important epidemiologic trends have influenced the approach to patients with suspected bacterial meningitis. One was a decline in invasive Haemophilus influenzae disease. A multistate surveillance study of bacterial meningitis occurring in 1986 in the United States identified H. influenzae as the pathogen in 47% of cases overall and 70% of those among children under five years old. H. influenzae type b conjugate vaccine was subsequently licensed for use in children 18 months of age and two months of age in 1987 and 1990, respectively. A second surveillance study of cases in 1995 identified H. influenzae as responsible for only 7% of cases overall, with only three cases reported among children under five years old (none of whom had received the vaccine). This 94% decline in the incidence of H. influenzae meningitis resulted in a marked decline in meningitis in children less than five years old and a shift in the median age of patients with bacterial meningitis from 15 months in 1986 to 25 years in 1995. With the virtual elimination of H. influenzae meningitis, S. pneumoniae has become the most common cause of bacterial meningitis among North Americans of all age groups beyond the neonatal period. Routine immunization of infants and children with the newly-approved pneumococcal heptavalent conjugate vaccine may result in a similar decrease in the incidence of pneumococcal meningitis.

The second epidemiologic trend was a dramatic global increase in infection with antibiotic-resistant strains of S. pneumoniae. The first case report of infection from a penicillin-resistant strain was in 1967; meningitis from such a strain was described in 1974. A sustained problem of clinically significant community-acquired infections in immunocompetent hosts with highly resistant strains of S. pneumoniae was described in South Africa in 1977. Over the past decade, there has been a worldwide increase in the incidence of infection due to S. pneumoniae resistant to penicillin and other beta-lactam antibiotics. In the United States, the prevalence of antibiotic-resistant strains of S. pneumoniae is as high as 35%, depending on the geographic region and age group examined. In a 1994 study from metropolitan Atlanta, for example, S. pneumoniae isolates from children and adults with invasive pneumococcal infection displayed significant levels of resistance to both penicillin (25% overall, 7% high-level) and cefotaxime (9% ov ...
