When confronted by Spanish soldiers and missionaries, they resisted violently. Those who did not succumb to the epidemics that swept over the isthmus either died fighting or fled to remote areas. A. The Colonial Period B. Nationhood Tobacco exports promoted the growth of a more prosperous society, and Costa Ricans became prominent in the intellectual and political life of Central America in the early 19th century. When Spanish rule ended in 1821, the country became part of Mexico until 1823, and then part of the United Provinces of Central America, from 1824 to 1838. However, it avoided involvement in the civil wars that plagued the latter federation. Costa Rican politics reflected the liberal-conservative ideologies found elsewhere in Latin America, with the towns of Cartago, San José, Heredia, and Alajuela vying for leadership. San José gained ascendancy, but the most important development of the mid-19th century was the growth of coffee as the major export.

At a time when all countries, including the United States, struggle to deliver equitable and affordable health care services that meet public needs and expectations, it is useful to see how other countries provide health care. This program examines the health care system in Costa Rica, a developing nation that has in many ways been successful in providing health care and other services. We will look historically and culturally at how its health care system came to be, and how it operates within the present political and social milieu. We will also examine from an ethical point of view the dilemmas that arise in providing health care effectively, efficiently and fairly, with limited resources. And we will compare how analogous issues arise and are dealt with in the United States. Students will be able to study and see first-hand how health care is being provided in a number of settings. We will also hear from a variety of local speakers who are health care professionals and people concerned with health care policy.

During his fourth voyage to the New World in 1502, Christopher Columbus landed at "Cariari," known today as Puerto Limon. Columbus actually named Costa Rica (rich coast) under the assumption that the land was filled with precious metals. The earth never yielded gold and silver, but the name was perfect for the wealth of natural beauty and flawless climate. The country has a territory of 51,000 km2 and a population of 5,000,000. Mountains spread from the northeast to the southeast forming a fertile central valley measuring approximately 3,000 km. At 4,000 foot elevation it has one of the most perfect climates in the world. The great majority of the population is concentrated here. The first settlers in Costa Rica were the Chorotega, Huetares and Brunca or Boruca Indians; today there are still about 20,000 Indians living here.

The development of a modern welfare state has resulted in Costa Rica's having high health indicators, a good standard of living, and high literacy rates. The conquistadors found a country with extremely rugged topography and sparse populations of indigenous people. Costa Rica was the poorest colony in the Spanish Empire. This in the long run was a blessing for the country, since the absence of precious metals and Indian labor force prevented the Spaniards from introducing semi-feudal institutions from the Iberian Peninsula into the province. Most of the natives escaped enslavement by fleeing into the Talamanca mountains, where they perished from wars of resistance, epidemics and wars among rival tribes. Extractions: Costa Rica ~ General History We'll begin our short trip through Costa Rica's history with pre-Colombian times. The famous Mayan empire did not extend this far south in Central America. The people here were called the Chorotegas . They farmed corn, beans, fruit, and cotton. They also hunted, had large cities with religious centers, and militaries for gaining land and slaves (used for labor and ritual sacrifice). The women were the only ones allowed access to internal commerce - the market. They were also the primary artisans and writers. Then Christopher Columbus landed at what is now Puerto Limon in 1502. Soon after, Spanish explorers began arriving, and as is the theme throughout Central and South America, the onslaught of the Spanish conquistadors greatly decimated the Chorotegas' numbers through war and disease. These indigenous peoples retreated to the highlands where they could, leaving the lowlands to the Spanish and those captured for slave labor. In 1562 Juan Vasquez de Coronado arrived to take charge of the new colony as governor, establishing the capital of Cartago on the rich volcanic soils of the central area. Due to Spain's out-of-sight-out-of-mind attitude toward its colonies, Costa Rica received little assistance from the father country and so had to subsist on its own, fostering a sense of pride, egalitarianism and self-sufficiency that exists to this day. The little tropical country split with Spain in 1821, moving its capital to San Jose. The 1830's brought the reign of the coffee industry. By the 1850's coffee caught on in Europe, replacing tea as the favored drink in many countries, bringing European influences and more money into the economy than Costa Rica had ever seen.

A mere 50 million years ago, this narrow mountainous strip of land was part of the ocean floor, along with the rest of the Central American isthmus between Guatemala and Colombia. Although that may sound like a long time back in earth's history, the dinosaurs had already disappeared some 15 million years previously. Currently, most geologists assume that from 40 million years ago until within only the past three million years what are now Costa Rica and its neighboring countries were then a volcanic archipelago. Using the theory of plate tectonics-which suggests that the earth's crust is fragmented in a number of sections that fit together like pieces of a jigsaw puzzle, though they are not stationary-the formation of Costa Rica's land mass is explained by its position at the western edge of the so-called Caribbean plate which is moving westward and overriding the Cocos plate located in the Pacific Ocean. As these two chunks of crustal material collide, the rocks of the Cocos plate are pushed downward, subjecting them to increased heat and pressure that eventually turns them into molten rock under great pressure. Because the downward thrust is at an angle to the east, when the pressure build-up finally becomes too much and the lava and steam move back upward towards the surface, they do so beneath the western edge of the Caribbean plate. The result is volcanism.
